DMS offers before and after school care along with a summer care program. Please select the appropriate registration forms below for information on pricing and times. For more information please contact the school office. Spaces are limited.

In order to register your child for any of our before or after school programs you must submit the following items to the school office:

-Completed registration form

-Certified copy of your child’s birth certificate or passport (unless already on file in the school office)

-Immunization record or waiver (unless already on file in the school office)

-Blue emergency card (a new form is required each year to ensure we have up-to-date information).

-First month’s payment in full
